The letter is to inform that all applications for registration of qualifications in the Malaysian Qualifications Register (MQR) from self-accrediting Higher Education Providers (HEP) are through submission of 'Form D: Application For Registration of Qualifications in the Malaysian Qualifications Register (MQR) and the Issuance of Accreditation Certificate'. The implementation of this form is effective from 12 January 2012 and can be downloaded from the MQA website at www.mqa.gov.my.
